[MACEP] permission forms for photos of students

Hi Everybody,
We are revising some very old (pre-Internet) permission forms (in English and Spanish) which we want to use for the inclusion of student photos in newletters and other publicity. I got to thinking that somebody--maybe even somebodies--out in macep-land ALREADY has something wonderful that they would be willing to share.
